EQUFSD Student Registration

Welcome to the East Quogue Union Free School District!  In order to ensure everyone’s health and safety, place students in the appropriate program, and conform to New York State Law and District Policy, the district requires that certain information and records be provided upon registration.

As of June 13, 2019,  New York State does not recognize religious and/or non-medical exemptions for immunization requirements. The only exemption recognized under the current law is a medical exemption.

IMPORTANT: The Registration Process is not complete until the registration form and all documentation is submitted and approved by the main office. Online registration enables parents to shorten the length of time required to register, however, documentation and certain forms still need to be processed in person.

  • Kindergarten-6th Grade: We are proud to offer our residents a new time-saving feature of our registration process - Online Registration!  Visit https://parentportal.eschooldata.com/eqsd and click Online Registration.  Note that if you already have a student in the district, and a portal account, please log into the portal, instead of clicking Online Registration.  Once in the portal, click Register New Student. This will save you some time during the online registration process. When an application is submitted you will be contacted by our main office and given information on how to complete the registration process. If you do not have access to a computer to complete the registration, please call the main office.  


EQ Registration Forms

According to NY State Law, in order to register your child/children in the School District, you must physically reside at your address within the East Quogue School District’s geographic boundaries.   All proofs of residency must be current, within 30 days of registration.

In addition to the completed online registration application, the following documents are required to register your child/children:

  • Student Documentation (1 of the following): Birth Certificate, Record of Baptism or Passport  (other documents may be considered if none of these are available).
  • Current Physical & Immunization Record
  • Proofs of Residency (Landlord Affidavit, if applicable)
  • Proof of Guardianship (if applicable)
